I hereby give United Way of Larimer County and all participating agencies permission to use photographs / videos of me. These photographs / videos may be used with or without identification of me by name in future publicity and communications of United Way of Larimer County and all participating agencies for any indefinite period of time.

I, for myself, and my heirs, executors, administrators and assigns, hereby release, waive and discharge United Way of Larimer County, its current and former employees, agents, successors, heirs, insurers, legal representatives and subsidiaries or other related entities from any and all past, present and future claims, causes of action, or claims for relief of any nature, known or unknown, related to use of photographs / videos of me.
